Order Form and Contact Information Sugar Shack Cupcakes will send you a confirmation email with 12 hrs after submission of your order.  Once the email has been sent your order is confirmed by Sugar Shack Cupcakes.  First Name: Last Name: Your Address: City: Zip Code: (5 digits)  Phone: Email: Delivery address: Date and time of pickup/delivery: Pick up or delivery ?: What would you like to order ?: Any special instructions/colors, etc.

Just a few thins to keep in mind when ordering:

* All orders require a minimum of 1 dozen and 1 flavor per dozen.
* Please place your order at least 48 hrs in advance - if you need them sooner, let us know and we will see what we can do.
* We can frost and fill with almost anything you desire, if you don't see the combination you want, just ask and we will try to accomodate you.
*  We do not have a retail location (yet), so all orders are pickup and delivery by appointment only.
*  All orders are onsidered final when Sugar Shack Cupcakes confirms the order with you.  If you do not receive an email from us within 12 hrs of your order, please contact us at 610-469-1759.
*  Orders over $100.00 require a deposit of 1/2 of the total order.
* You may not cancel your order within 24 hrs of scheduled delivery/pickup date and time.
